
The All Hearts Collaborative is a partnership with three non-profit organizations–the Hypertrophic Cardiomyopathy Association (HCMA), the Preventive Cardiovascular Nurses Association (PCNA).
The Collaborative seeks support for an initiative to reduce cardiac healthcare disparities impacting Black and urban Americans by addressing the lack of medical practitioners and patient education about familial heart disease in underserved communities.
Our project will work with urban churches in primarily Black communities to increase patient knowledge of the importance of adhering to hypertension treatment plans and speaking to their physician about their family's heart history.
It will also increase access to quality healthcare in these communities by educating community healthcare teams about the differential diagnosis of left ventricular hypertrophy, HCM, Amyloidosis, Fabry Disease, HFpEF, and other diagnoses.
Through the HCMA's Lori Fund, it will also provide referral patterns and travel support for patients who require specialized cardiac care.
