- Support for the costs of daily life for HCM Hospice patients, such as rent or food.
How do you qualify for the Lori Fund?
Those who earn up to 300% of the Federal Poverty Guidelines based on household size are eligible for the Lori Fund. Income verification is required. You must complete the application and submit income verification, such as W2s, pay stubs, or social security statements; other documents may be accepted.
How can I apply for the Lori fund?

- While completing the Lori Fund application, you may also apply for a Membership Scholarship, which allows you to take advantage of the perks of an HCMA membership for one year.
- Allow a minimum of 14 days for processing.
- We can help you schedule free air travel through our partner Angel Flight.
