Intake and Navigation for International Clients


To our International friends who wish to set up an Intake with the HCMA, we are thrilled to connect with you! We can provide services to support your needs. During the Intake, we collect information to understand your condition and situation. During the Navigation Call, we answer your questions and provide guidance. 


Here's how to take the next steps:


Step 1: Schedule an appointment

Go to https://4hcm.org/education-and-support/. Under the photo of each Intake Coordinator, you will find a link to their calendar. Clicking the link will only display the calendar for the selected person. The calendar will show you available appointments in your local time zone. If you don't see an appointment that suits your schedule, try a different person's calendar. If you can't find an appointment that works for you, please email support@4hcm.org for assistance.

 

Once you've found a suitable appointment time, click on it to book. Complete your contact details and request a Zoom appointment to avoid international calling costs. 


Following your booking, please review the confirmation email carefully, as it contains essential information about the appointment duration and necessary preparations. Being prepared will ensure that your appointment runs smoothly. If English is not your first language, consider using Google Translate to help you understand the confirmation email.


Do you need translation services?

You can bring your own translator/interpreter or use our paid translation service. If you choose your own, you will manage any details and costs directly. If you would like to use an interpreter we hire, we kindly request that you provide a credit card to cover the expense. At this time, the service we have identified charges $2.00/minute for spoken Spanish and $2.50/minute for other spoken languages. 


Punctuality is important, especially when we have arranged a translator. We advise verifying that your confirmation lists the appointment time in the correct time zone for your location.


Step 2: Send your medical records as PDFs to your Intake Coordinator.
 

Typically, we request Resting Echocardiogram, Cardiac MRI, Stress Echocardiogram, EKG, and Holter Monitor results. You do not need to provide your entire medical record; just one, most recent copy of each test will do. We do not need images, just a written report. If your test reports are not in English, we appreciate your efforts in translating them so we can understand them.


Each PDF file should contain its own report. Do not put all reports into one PDF. It's best to send one email with all reports as attachments in that email. You may send your reports in advance if you choose - this can save time in scheduling your Navigation Call.


Following your Intake, once we have received your medical records, we will arrange a second appointment for a Navigation Call. This call is designed to address any questions you may have and provide guidance for next steps. The Navigation Calls are 30 minutes for paid HCMA members and 15 minutes for non-members.
