What is the Lori Fund?
The HCMA's Lori Fund provides grants of up to $600 per year to HCM patients with financial needs.
What can funds be used for?
Funds can be used for:
- Travel costs to an HCMA Recognized Center of Excellence (COE) for HCM care, including transportation, meals, and hotels.
- Costs for an HCM patient traveling to a facility for heart transplant care, such as transportation, meals, and hotels.
- End-of-life costs for HCM patients, such as cremation or burial.
- Support for the costs of daily life for HCM Hospice patients, such as rent or food.
How do you qualify for the Lori Fund?
Grants are available for those who earn up to 300% of the Federal Poverty Guidelines based on household size. Income verification is required. You must complete the application and submit income verification, such as W2, paystub, or social security statement; other documents may be accepted.
How can I apply for the Lori fund?
You must first complete our
Intake and Navigation Call process before you apply.

Lori Fund Scholarship.
While completing the Lori Fund application, you may also apply for a
Membership Scholarship, which allows you to take advantage of the perks of an HCMA membership for one year.
