The attached PDF (to the left on your screen) document its a simple and easy to use tool to help identify those at risk for conditions that can cause sudden cardiac arrest in the young (defined as under 50). This tool is NOT specific to HCM, it looks for risk factors for a wide variety of cardiac abnormalities.
If you answer "unsure" or "yes" to one or more of the questions the HCMA recommends you discuss the form with your primary care physician and seek additional cardiac evaluation by a cardiologist for your child or yourself.
